The following medical insurance coverage plans are accepted:

• HMSA (PPO, HMO, QUEST)

• HMAA

• UHA

• PSWA (AFL Unite Here Local 5)

 

Co-payment and applicable Hawaii GE tax are collected at the beginning of each session in cash, check or credit card. For returned checks a $40 fee will be applied.

For "no call/no show" or cancellations with less than 24 hours notice, you will be charged a $100 fee as this is not covered by insurance.

Please be advised that not all clinical services are covered by every insurance provider, it is important that you find out exactly what mental health services your insurance policy covers at the outset of therapy. It is the responsibility of the client to check with your insurance company on coverage and copayments/deductibles.
